Job summary

Nottinghamshire Healthcare NHS Foundation Trust is offering an exciting specialist outreach role for dementia care in residential/nursing settings. You will provide specialist nursing advice, treatment and education under the guidance of a registered team, contributing to assessments and interventions within the dementia outreach service.

You will engage with the Enhanced Health in Care Homes Service (EHCH) and local GPs, working in a multi-disciplinary team to raise care standards and support

Qualifications

• Experience in dementia care or related field.
• Registered nurse or allied health professional.
• Willingness to work with a multi-disciplinary team and care homes.

Responsibilities

• Provide specialist nursing/healthcare advice and treatment for clients with dementia.
• Contribute to assessment and interventions within dementia outreach.
• Engage with EHCH service and local GPs.
