Job summary

Nottinghamshire Healthcare NHS Foundation Trust is recruiting a Dementia Outreach Support Worker to join the Dementia Outreach Service in Gamston. The role involves supporting care homes in Rushcliffe, implementing patient care plans and providing specialist nursing guidance under registered team members.

You will monitor mental health status, help residents develop coping strategies, and collaborate with a multidisciplinary team.

Qualifications

• A role within NHS dementia outreach services.
• Multidisciplinary teamwork and patient-facing skills.

Responsibilities

• Support the Dementia Outreach Service with patient care plans and guidance for care homes in Rushcliffe.
• Monitor and record individuals' mental health status and report significant changes.
• Assist individuals in developing coping strategies to manage symptoms.
• Treat patients who are permanent residents in care homes in the Rushcliffe area.
• Make and respond to care home staff telephone calls for support and guidance; review electronic records and document entries.
• Attend meetings in person and via MS Teams.
• Be a member of a multidisciplinary Team.
