About the Role

CJP Outreach Services is looking for caring, motivated and enthusiastic female support workers to join a brand-new support team supporting a 48-year-old woman as she transitions from a long-term inpatient placement into her new home in Wellington, Somerset.

This is an exciting opportunity to be part of a new team supporting someone to take the next steps in her journey and rebuild a meaningful, fulfilling and independent life in the local community.

R is engaging, charming and, at times, a little shy. She is excited about returning to living in the community and building a life beyond hospital. R has a range of interests, including gardening, cooking, make-up, beauty and dog walking, and we want to build on these interests to help her develop a positive routine and reconnect with the things she enjoys.

R will be supported on a 1:1 basis by a consistent female support worker team. This will help build consistency, reassurance and the relationships she needs to feel safe and confident in her new home.

Importantly, this isn't simply about doing things for R. It is about working alongside her, encouraging her to develop and maintain skills and supporting her to have as much choice and control over her life as possible.

You will work as part of a dynamic, resilient team that values diversity, promotes inclusion, and encourages a positive and supportive working environment. Each female support worker will play a key role in helping R achieve her goals and aspirations.

About You

We are looking for people with experience of supporting individuals with mental health needs, including schizophrenia and psychotic illness, and who understand how these conditions can affect a person's cognition, insight, decision-making, emotional regulation and ability to engage with support.

As a female support worker, you will ideally have experience working within community-based settings, such as supported living or residential care. Previous experience in mental health, complex care and/or rehabilitation services would be an advantage.

You will need to be confident in recognising changes in presentation and understanding that a person's ability to engage, make decisions and manage everyday tasks can fluctuate depending on their mental health. A female support worker will be able to remain calm, patient and consistent, particularly when R is distressed or finding situations difficult.

Knowledge and experience of Positive Behaviour Support (PBS) would be an advantage, as would an understanding of the impact of developmental trauma and experience of working in a trauma-informed way.

Most importantly, we are looking for female support workers who can see beyond R's diagnosis and recognise her strengths, interests and potential.

Staff with a full clean driving licence are preferred.

Full DBS check required as part of the role.

  • Develop a positive daily routine to help R settle into her new home.

  • Support with everyday skills such as cooking, personal care, budgeting, and household tasks.
  • Support health and wellbeing, including medication, medical appointments, and healthy lifestyle choices.
  • Encourage social inclusion by supporting access to community activities and building meaningful relationships.

  • Keep accurate records of progress, outcomes, and daily activities.
  • Collaborate with colleagues, families, and professionals to ensure consistent, holistic support.

Hours

Support is provided on a 24/7 basis with a staffing ratio of 1:1.

You will work on a rotational rota basis covering 15-hour shifts, including every other weekend. The role will also include sleep-in duties.

R currently lives in Exeter but is due to move to her Wellington accommodation shortly. You are likely to be required to support with transition visits as a female support worker.
