About the role

The role of the Support Worker is to provide specialist expertise and support to facilitate teams in achieving person-centred outcomes for the people we support, some of whom have profound and complex needs, to maximise the individual's independence and enable them to be active in their communities.

The service provides accommodation-based support for women experiencing mental health challenges. The people we support are encouraged to develop and maintain their independence while receiving support in areas where they may need additional help. This can include managing benefits, self-medication, cooking, budgeting, maintaining relationships with family and professionals, looking after their health and wellbeing, and engaging with their local community.

Support is tailored to each person's individual needs and focuses on helping them feel safe, well, and confident enough to progress in their recovery. The overall aim is to build independence and support women to move on successfully to the least restrictive setting that meets their needs.

Duties will include:

  • Making a difference in the lives of those you support
  • Helping to use a range of recovery-oriented and person-centred planning tools
  • Ensuring all plans and records relating to the people we support are accurate and up to date
