Job summary

Action for Children in Wales is seeking a Family Support Practitioner to help families access local services, attend parenting programmes and build resilience. The role involves working in families' homes and communities across Ceredigion to provide practical and emotional support tailored to each family's needs.

You will manage a caseload, complete assessments and maintain records, while building strong relationships with partners to ensure coordinated support.

Qualifications

• Experience with children, young people and families in a support, education, health or community setting.
• Strong assessment, planning and case management skills.
• Ability to build positive relationships and engage families facing a range of challenges.
• Good written and verbal communication skills, with the ability to maintain accurate records.
• Understanding of safeguarding, confidentiality and professional boundaries.

Responsibilities

• Manage a caseload of families and ensure support is delivered in line with plans.
• Complete assessments, reviews and accurate case records to monitor progress.
• Build and maintain effective relationships with partner agencies for coordinated support.
• Safeguard and promote the welfare of children and respond to concerns and risks.
• Signpost and refer families to relevant services to access needed support.
