Job summary

Pact is seeking a Personal Wellbeing Manager (Family & Relationships) to support service users, families and carers within a multi-disciplinary team in England and Wales. You will deliver one-to-one and group wellbeing work, focusing on positive relationships, resilience and desistance from offending.

The role involves risk-aware, person-centred planning, strong liaison with Probation and partners, and evidence-based record-keeping to demonstrate outcomes.

Qualifications

• Experience of working with people who have multiple or complex needs and achieving outcomes.
• Ability to deliver family work and engage with diverse groups.
• Knowledge of safeguarding children and vulnerable adults best practice.
• Strong engagement skills with empathy, rapport and boundaries.

Responsibilities

• Provide welcoming, safe and human support with respect and dignity.
• Act as the family relationships expert within the Personal Wellbeing service.
• Assess referred service users and agree bespoke, person-centred action plans.
• Deliver intensive one-to-one and group interventions and advocacy with statutory and third-sector services.
• Maintain strong operational relationships with local Probation staff and partner agencies.
• Monitor Risk of Serious Harm indicators and de-escalate when needed to keep people safe.
• Record activity using digital systems and share information appropriately with partners.
• Contribute to meetings, case conferences and continuous service improvement.
