Job summary

Somerset County Council is seeking an Assistant Practitioner (Healthy Families) to join our Public Health Nursing Service in Somerset. You will deliver early intervention and health promotion to children, young people and families, under supervision, across community settings including Family Hubs and schools.

You will work with Health Visitors and School Nurses, contributing to assessments, care plans and safeguarding while building strong, family-centred relationships to improve outcomes and

Qualifications

• Level 4 qualification (or equivalent) in a relevant field.
• Level 2 English and Maths are required.
• Experience working with children, families or in health serving settings is advantageous.
