Job summary

We have an exciting new post for the Falls Prevention Brighton and Hove team, part time 24 hours per week. This role involves a variety of duties as per below:

  1. a) To identify and carry out the specialist assessment of individuals who may be at risk of falls and fractures.
  2. b) To plan and provide programmes of care according to a person's individual and complex needs and without direct supervision in a variety of settings, including clinics, exercise groups, health promotion, and education.
  3. c) To manage a caseload of patients utilising specialist clinical assessment, diagnostic, and treatment skills in the management of patients at risk of falls.
  4. d) To provide training and education on falls prevention to a broad range of staff working across the local health and social care sector, including Residential and Home Care Services, and voluntary organisations.
  5. e) To supervise assistant staff/support workers and undergraduate students and provide training to staff and carers.
  6. f) To develop strong links with the Primary Care Trust, Sussex Ambulance, the Voluntary and Independent Sector, and the local community to deliver an effective service.
  7. g) To adhere to professional standards and guidelines of practice, education, and conduct.

Main duties include supporting the Falls and Fracture Prevention Service Manager in monitoring and evaluating the service, carrying out clinical audits, maintaining data entry, developing links with relevant specialists and organizations, liaising with consultant Geriatricians, and supporting service development including care pathways, policies, and procedures.
