Duties
We have an exciting new post for the Falls Prevention Brighton and Hove team, part-time 24 hours per week. This role involves a variety of duties, including:
- Identifying and carrying out specialist assessments of individuals at risk of falls and fractures.
- Planning and providing care programs tailored to individuals' complex needs in various settings such as clinics, exercise groups, and health promotion activities.
- Managing a caseload of patients using specialist clinical assessment, diagnostic, and treatment skills for fall risk management.
- Providing training and education on falls prevention to staff across the local health and social care sectors, including residential, home care, and voluntary organizations.
- Supervising assistant staff/support workers and undergraduate students, and providing training to staff and carers.
- Developing strong links with the Primary Care Trust, Sussex Ambulance, voluntary and independent sectors, and the local community to deliver effective services.
- Adhering to professional standards and guidelines of practice, education, and conduct.
Main Tasks
- Supporting the Falls and Fracture Prevention Service Manager in monitoring and evaluating the Falls Service in Brighton and Hove, according to organizational targets.
- Participating in clinical audits as part of the team’s effectiveness plan.
- Maintaining up-to-date data entry following Trust policies, including activity/contact data using systems like PIMS/DISCO.
- Building links with Fracture Prevention Specialists, Geriatricians, and the Royal Osteoporosis Society for coordinated service delivery.
- Liaising with consultant Geriatricians regarding diagnostic falls clinics and managing referrals.
- Carrying out additional tasks as directed by the Osteoporosis and Falls Prevention Service Lead.
- Supporting service development, including care pathways, policies, and procedures aligned with the Calderdale Framework.
