Job summary

The Willow Group in Gosport is seeking an experienced Extended Clinical Practitioner to join our GP group. You will work autonomously, running clinics and supporting our ongoing primary care initiatives.

You will lead nursing care, collaborate with the general practice team and the extended primary care network, and contribute to policy delivery and new care models for our patient population.

• Experienced nurse/AHP, working within professional boundaries.
• Ability to work autonomously.

Job description

We have an exciting opportunity for an experienced Extended Clinical Practitioner to join to The Willow Group, a forward thinking and innovative GP group providing primary care services for 49,000 patients over 5 local sites in Gosport.

We are looking for an enthusiastic, caring and motivated professional with excellent clinical, organisational & communication skills, who has the ability to work autonomously.

The post holder will be an experienced nurse/AHP, acting within their professional boundaries, to support our on-going initiatives. The post holder will also hold their own clinics and demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team and extended primary care team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership. They will look to develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice population.
