Job summary

Windsor House Group Practice is seeking an experienced Practice Nurse to join the team for 2.5 days per week in the Leeds area. The post involves working in a large training practice with approximately 20,000 patients, delivering high-quality care and leading on clinical targets.

The successful candidate will collaborate with Partners, GPs, nurses, registrars and HCAs, undertake a range of nursing duties, and promote professional development and evidence-based practice within the team.

Qualifications

• Experience of working within a general practice.
• Understanding or experience of audits.
• Experience of managing clinics in a Primary Care setting.
• Experience in the development of General Practice services.

Responsibilities

• Assist GPs to help deliver effective management of patients with chronic diseases as detailed by the Quality and Outcomes Framework.
• Contribute to the development of policies/protocols/guidelines within the practice.
• Aid the practice team with implementation of new initiatives in chronic disease management services.
• Formulate reports as required for the practice management.
• Support the implementation of evidence-based guidelines with the nursing team.
• Encourage development of integrated Primary Care services and multidisciplinary working.
