Job description

This is a great opportunity requiring compassion, empathy, and organisation. The role is to work closely with the Multi-professional Team (MDT), and under the supervision and guidance of the Coordination and Engagement Manager to ensure patient engagement with the delivery of care to Children and Young People (CYP) and their families. This will involve being amongst the family’s first contact with the service, and an ongoing bridge between them and the Multi-professional team of clinicians and managers involved in their care.

The postholder will be required to work up to 37.5 hours per week.

Responsibilities:
  1. Manage the daily operations of the Engagement Team, including workload, staff development, wellbeing, peer review, and supervision.
  2. Case manage and serve as the point of contact for patients and families.
  3. Coordinate support and liaise effectively with all involved professions, agencies, and parties.
  4. Promote and facilitate timely attendance to appointments.
  5. Lead virtual, telephone, and face-to-face appointments with families.
  6. Travel to various clinic locations across the South West.
  7. Provide emotional support to patients and families, including during crises.
  8. Offer information about further support tailored to the patient’s needs.
  9. Be aware of the limits of own practice and knowledge, seeking support or advice when needed.
  10. Document and monitor all aspects of care coordination and service delivery, supporting data collection for audits (including Clinical Document System, Careflow EPR, Evolve, and other data sources).
