End-of-Life Home Care Assistant – Flexible 30h

Job summary

Cornwall Partnership NHS Foundation Trust in St Austell is offering two Community Healthcare Assistant posts at Band 3 on a permanent contract. Each post is 30 hours per week, covering weekends and evenings on a rota, with mileage paid when travelling to patients’ homes.

The role provides end-of-life and at-home care as part of the Continuing Healthcare at Home service, requiring compassionate, patient-centred care and the ability to work alone or with nursing colleagues.

Qualifications

• NVQ Level 3 qualification in care or equivalent experience.
• Trained in Venepuncture or willingness to undertake relevant training.
• Evidence of experience in care setting

Responsibilities

• You must have access to your own transport (mileage paid).
• Be able to work over a 7 day period, 365 days per year on a rota basis.
• Work on your own as well as part of the wider team with support from registered nursing staff.
• Provide high quality holistic care focused on what matters to the service user, with compassion.
• Some training is provided; knowledge and skills in this area are preferred.

Education
