Sheffield City Council seeks a Provider Services Worker to join its Enablement Team. You will support adults to build skills, confidence and independence through time-limited, person-centred enablement, working in homes, workplaces and community settings.

The role involves city-wide travel and lone working with flexible hours; applicants should have experience in enablement for learning disabilities, physical disabilities or autism, and be able to work independently after induction.

• Supporting access to volunteering, employment and community opportunities.
• Delivering support in accordance with person-centred enablement plans, agreed outcomes and risk-management arrangements.
• Monitoring progress, maintaining accurate records and contributing to reviews of enablement outcomes.
• Working independently, using professional judgement and contributing positively to a supportive multidisciplinary service.
