Job summary

Powys County Council is seeking an Enablement and Care Support Worker in Brecon to implement Personal Plans and support individuals to regain skills and live independently in their own homes. You will work to keep people living safely in the community and meet the requirements of the Care Inspectorate for Wales.

The role is full-time, permanent, field-based, located in Aberhonddu with a salary of £27,274 to £28,153 per year.

Qualifications

• Enhanced DBS check required.
• Experience in community/home care is preferred.

Responsibilities

• Provide front line care and support to individuals in their homes.
• Promote independence to help individuals achieve personal outcomes.
• Work with colleagues and other professionals to support wellbeing and community presence.
• Complete required documentation in line with policies.
• Pursue ongoing professional development in line with Social Care Wales.
• Attend team meetings and training sessions.

To assist or enable service users to remain living within the community, enhancing the quality of their lives, supporting and maintaining them safely in their own environment. Complying with

requirements of a service Registered with Care Inspectorate for Wales under Registration and Inspection of Social Care Act 2016.

• To ensure continued professional development in line with agreed ways of working and Social Care Wales.
• Engage and participate in team meeting / training and engagement sessions.
