Responsibilities

• Support service users with mental illness and complex needs to live as independently as possible
• Assist with daily living activities and community participation
• Transport service users between the service and activities ensuring safety
• Follow care plans, risk assessments, and safeguarding procedures
• Attend handovers, meetings, and supervision as required
• Maintain accurate records of service users in the electronic patient record system Ablyss
