Job summary

Barnet Council is seeking a qualified social worker to deliver high-quality support for adults and their carers under the Care Act 2014. You will assess needs, develop person-centred plans, and safeguard those at risk, ensuring well-being and outcomes are at the core of every intervention.

You will collaborate with multidisciplinary teams, keep robust records, and signpost to local services to promote resilience and independence within the community.

Qualifications

• Qualified social worker with registration in Social Work England.
• Understanding of social and health care within the community.
• Ability to conduct robust assessments and risk assessments.

Responsibilities

• Provide high standard of social work to adults and/or carers under Care Act 2014.
• Undertake and review individualised and outcomes-led care and support planning.
• Safeguard adults and carers at risk of harm and promote safeguarding principles.
• Signpost to local preventative services, promoting resilience and independence.
