Together, we build a caring community where the warmth we show one another shines through in every life we touch.

Job overview

We have an exciting 12-month fixed-term opportunity for an Emergency Care Practitioner to join our integrated Urgent Community Response (UCR) and Hospital at Home (H@H) services, providing high-quality clinical care to people with acute, complex and undifferentiated health needs in community settings.

You will use advanced assessment, diagnostic reasoning and decision-making skills to assess, diagnose, treat and manage acutely unwell patients, helping to prevent avoidable hospital admissions and support early discharge. You will work autonomously while collaborating with multidisciplinary colleagues across primary, community, acute and social care.

As a senior clinician, you will provide clinical leadership, support safe caseload management and contribute to service development and quality improvement.

The role is across 7 days and includes rotational shifts between 08:00 and 23:00.

You must be a Registered Nurse with current NMC registration or a Paramedic with current HCPC registration.

As this is a community-based role, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

As an Emergency Care Practitioner, you will:

  • Assess referrals, prioritise patients according to clinical need and urgency, and respond within agreed timescales.
  • Use advanced clinical assessment, diagnostic skills and expert knowledge to identify physical and psychosocial needs and develop short-term care plans that optimise health and wellbeing and help prevent avoidable hospital attendance or admission.
  • Deliver advanced, autonomous clinical care to patients with acute, undiagnosed or complex health needs, long-term conditions and red-flag presentations.
  • Undertake face-to-face assessments and interventions in patients’ homes, including transporting and handling clinical equipment safely.
  • Provide clinical leadership, support caseload prioritisation and risk management, and offer supervision and guidance to junior colleagues.
  • Work collaboratively with multidisciplinary teams to support safe, joined-up patient pathways and continuity of care.
