The role involves supporting a 77-year-old client with dementia, memory issues and mobility, helping to establish routines and provide prompts, while ensuring safety and well-being. Applicants must hold a full driving licence and undergo a fully enhanced DBS check funded by the employer.

Qualifications

• Must have full driving licence.
• Fully enhanced DBS clearance required and funded by employer.
• Registration with DBS Update Service.
• Westcountry Case Management safeguarding policy commitment.

Responsibilities

• Provide support to establish and maintain structured weekly routines for the client.
• Assist with daily living tasks and monitor safety in the home.
• Support with emotional well-being and behavior management as required.
• Coordinate with therapists and carers for reviews and appointments as needed.
