Job summary

Stoke-on-Trent City Council is seeking an Early Years Inclusion Officer to enable young children aged 0 to 5 with SEND to learn, develop and flourish. The role involves planning inclusive programmes via Portage, home visits and stay-and-play sessions, and multi-agency collaboration with families and professionals.

You will identify SEND early, support timely interventions, maintain up-to-date assessments, and provide practical guidance and reports to colleagues.

Responsibilities

• Plan and deliver inclusive early years programmes through Portage groups, home visits and stay and play sessions
• Support the planning and delivery of multi-agency assessment activities
• Contribute to service improvement by reviewing provision, identifying gaps and supporting new initiatives
• Provide advice, guidance and practical strategies to support children with SEND and specific disabilities
• Identify SEND early in line with the graduated approach, ensuring timely intervention and reducing delays in accessing appropriate support
• Maintain accurate assessments and development records for individual children and record information on the service database
• Communicate effectively to build positive relationships with families and professionals and produce clear reports and professional advice
