Job summary

East Midlands Councils in High Peak is seeking a Family Help 0-5 Practitioner to work directly with children aged 0-5 and their families to strengthen parenting capacity, support development and readiness for school.

You will work alongside families and partner agencies, delivering practical, evidence-based early-years interventions, parenting programmes, and signposting to parents and carers, while maintaining high-quality records and contributing to safeguarding processes.

Responsibilities

• Working with families and partner agencies to develop clear action plans with measurable outcomes.
• Assessing and tracking children's development and progress, including readiness for school.
• Delivering targeted, evidence-based early-years interventions for children who may be at risk of developmental delay.
• Delivering parenting programmes.
• Providing advice, guidance and signposting to parents and carers.
• Maintaining high-quality records and reports.
• Contributing to safeguarding processes and multi-agency plans that help children remain safe and achieve positive outcomes.
