Derbyshire County Council is seeking a Best Start Inclusion Practitioner to support families with children aged 0–5 across our Best Start Family Hub network. You will provide early identification, family navigation and joined-up support in collaboration with health services, early years providers and community partners.

The role focuses on accessible, hands-on early support, helping families access the right services at the right time and championing inclusion across communities.

Formación

• Experience working with SEND and inclusion, and understanding child development.
• Experience supporting families in community, early years or similar settings.

Responsabilidades

• Early Identification: identify developmental or emerging needs through observations and family engagement.
• Family Navigation: help families understand and access local services, including inclusion pathways and resources.
• Joined-Up Working: collaborate with health services, early years providers and SEND teams to provide coordinated support.
• Direct Support: offer drop-ins, short workshops and family sessions at Family Hubs and in the community.
