Job summary

The Methodist Church in Britain is seeking a Care Coordinator for a six-month maternity cover in the CAMEO team serving Peterborough, Fenland and Huntingdon. The role involves building care plans, assessing risk, and working with families, carers and partner agencies.

It is a hybrid post requiring travel across Cambridgeshire and supervision of staff and students. Registration in nursing, social work or occupational therapy is required, and visa sponsorship is not available.

Qualifications

• Evidence of continuous professional development.
• Registration with a recognised regulator (NMC, Social Work England, or HCPC) related to nursing, social work or occupational therapy.

Responsibilities

• Develop care and recovery plans with service users within NICE/quality standards.
• Assess individuals and families, including those at higher risk.
• Provide specialist advice from your professional background.
• Assess and manage ongoing risk, reviewing factors with service users and carers.
• Deliver specialist interventions including outreach, psycho-education and discharge planning.
• Collaborate with partner agencies to support social inclusion and recovery.
• Offer specialist interventions to carers and families.
• Promote safety and wellbeing of service users, families and carers.
• Provide clinical supervision to staff and students.
• Apply early intervention ethos, including work with under-17s when required.
• Travel independently around the county to meet deadlines.
