An established industry player in mental health services is seeking compassionate Early Discharge Coordinators to enhance patient support and facilitate smooth discharges. This role is crucial for providing high-quality care alternatives to hospital admissions. Due to expansion within the Stockport Home Treatment Team, we are excited to offer 2 full-time positions for Early Discharge Coordinators.
Main Base: Stepping Hill Hospital.
Open to: Nurses, Occupational Therapists, or Social Workers.
The Stockport Home Treatment Team is a multidisciplinary team including a Consultant Psychiatrist, junior doctors, Team Manager, Deputy Manager, Psychologist, NMPs, Lead Nurses, Occupational Therapist, Social Workers, Support Workers, Pharmacy Technician, and Administrative staff.
We are piloting a new role within the team to enhance support on acute wards and facilitate early discharge to the Home Treatment Team. The postholder will collaborate closely with both the Home Treatment Team and acute wards to ensure smooth patient discharges.
This well-established team provides high-quality assessment and home treatment services as alternatives to hospital admission for individuals experiencing acute mental health crises. We are aligning our services with the Fidelity model for Crisis services.
The role involves participation in ward rounds, coordinating patient discharges, and working with assessment teams to improve care. We seek compassionate, engaging individuals who prioritize person-centered care, respect, and dignity, and who can work effectively under pressure in a diverse, fast-paced environment.
Applicants should have experience as a qualified mental health practitioner and demonstrate strong leadership skills.
