We are piloting this new role within the team to enhance support on acute wards and support early discharge to the Home Treatment Team.

We are working to enhance our team in line with the Fidelity model for Crisis services, collaborating with all HTTs.

You will work within the MDT, maintaining close links with ward consultants and the Psychiatrist and Psychologist from the Home Treatment Team.

You will participate in ward rounds, coordinate patient discharges, and collaborate with assessment services to improve care provision.

We seek an individual who demonstrates compassion, warmth, engagement, and person-centered care focus.

You should respect diversity, work well under pressure, have excellent communication skills, and experience as a qualified mental health practitioner with strong leadership skills.

About us

We provide high-quality mental health and learning disability services across Greater Manchester's five boroughs—Bury, Oldham, Rochdale, Stockport, and Tameside & Glossop.

Our vision is for a happier, more hopeful life for our communities, and our staff are committed to delivering excellent care. We value our #PennineCarePeople and strive to be a great place to work.

We encourage applications from all backgrounds, including those with lived experience of mental illness or as carers.

Benefits include generous annual leave, flexible working, CPD access, involvement in research, wellbeing activities, and staff discounts.
