An exciting opportunity for 2 full-time Early Discharge Coordinators at a leading mental health service in Stockport. The role involves working closely with multidisciplinary teams to facilitate smooth discharges from hospitals, ensuring high-quality care for individuals experiencing acute mental health crises. Due to expansion within the Stockport Home Treatment Team, there is an exciting opportunity for 2 full-time Early Discharge Coordinators.
Your main base will be at Stepping Hill Hospital.
This post is open to Nurses, Occupational Therapists, or Social Workers.
The Stockport Home Treatment Team is a large multidisciplinary team, including a Consultant Psychiatrist, junior doctors, Team Manager, Deputy Manager, Psychologist, NMPs, Lead Nurses, Occupational Therapist, Social Workers, Support Workers, Pharmacy Technician, and Administration staff.
We are piloting this new role within the team to enhance support on acute wards and support early discharge to the Home Treatment Team.
The postholder will work closely with both the Home Treatment Team and the acute wards at Stockport to facilitate smooth discharges from hospital.
This well-established, multidisciplinary team offers high-quality assessment and home treatment services as alternatives to hospital admission for individuals experiencing acute mental health crises.
We are working to enhance our team in line with the Fidelity model for Crisis services, collaborating with all HTTs.
You will work within the MDT, maintaining close links with ward consultants and the Psychiatrist and Psychologist from the Home Treatment Team.
You will participate in ward rounds, coordinate patient discharges, and collaborate with assessment services to improve care provision.
We seek an individual who demonstrates compassion, warmth, engagement, and person-centered care focus.
You should respect diversity, work well under pressure, have excellent communication skills, and experience as a qualified mental health practitioner with strong leadership skills.

Main Duties and Responsibilities
Support admission and delayed discharge activities within acute inpatient services, attend weekly delayed discharge meetings, ensure S117 processes are followed, and facilitate community placements and funding applications.
Minimize bed crises by managing discharges and placements, develop effective discharge processes, and liaise with key stakeholders like ICB and local authorities.
Maintain accurate records, participate in MDT meetings, and support staff training and supervision.
Ensure compliance with policies and stay updated on clinical competencies.
Assist patients on home leave and during transitions to Home Treatment or crisis accommodation.
