This is an opportunity to be part of a team that values your experience and dedication! Our vibrant Children and Young People Disability team, within our Outstanding Children's services, seeks a passionate and experienced Social Worker.

Main duties of the job

We are looking for someone excited to contribute to our innovative service, where children, young people, and families are at the forefront.

Operating across Wiltshire's picturesque landscapes, our 0-18 disability social work service aims to make a positive impact on those requiring ongoing support.

As a key team member, you'll establish strong relationships within children's services and the 18-25 transition service, facilitating seamless transitions. The role involves regular travel across the county to provide accessible support where needed.

We require an experienced registered Social Worker with a thorough understanding of children in care. Up-to-date knowledge of relevant legislation and guidance, safeguarding, assessments, and chairing meetings including Adulthood preparation is essential.

Job description
Responsibilities include
  • Undertaking person-centred assessments for children and young adults, planning support/services to help them meet their outcomes and live independently.
  • Ensuring accurate, timely recording of all assessment activities in social care records.
  • Being the primary worker for Children in Need, Children Looked After, and children needing protection, depending on team function.
  • Operating within county policies, staying updated on social work methods, legislation (Children’s Act 2004, Children & Families Act 2014, Care Act 2014, etc.), and safeguarding procedures.
  • Interpreting statutory duties and departmental policies in service provision.
  • Managing allocated work based on eligibility, with case management responsibility.
  • Understanding assessment and risk management for adults, supporting customers to manage risks per safeguarding policies.
  • Completing enquiries, assessments, and safeguarding for vulnerable children and adults, considering their views and wishes.
  • Developing and implementing plans with service users and families, reviewing and revising as needed.
  • Convening and participating in meetings like Core Groups, writing reports, and attending legal proceedings.
  • Participating in duty rotas and engaging in professional development and supervision.
  • Traveling daily to visit families in their homes or other settings.
