An exciting opportunity has arisen to join our multidisciplinary community team as a Diabetes Nurse within the Doncaster Community Diabetes Specialist Nursing Team.
The Diabetes Specialist Nursing service is part of RDaSH, a forward-thinking organization that supports development and innovation to enhance patient care and service development.
The aim of the service is to promote self-care among patients, avoid unnecessary hospital admissions, and support timely discharges to maximize independence at home.
The successful candidate will possess excellent communication, organizational, and problem-solving skills, and will effectively manage their workload under the supervision of the Diabetes Specialist Nurses.
Applicants must demonstrate competence and confidence in a broad range of relevant clinical skills and knowledge.
The diabetes nurse is a qualified, experienced nurse with specific training in diabetes mellitus and its management.
Under the supervision of the Diabetes Nurse Specialist, the nurse will work across settings to deliver specialist clinical diabetes services to adults.
Provide interdisciplinary education and training to staff and students.
Contribute to service development both locally and nationally.
Participate in clinical audits within their area.
Maintain high standards of professional care.
