We are looking for an enthusiastic and adaptable Caseload Manager to join our caring and progressive team in Here Bay.
You will work as part of a close-knit team dedicated to the health and wellbeing of people in Here Bay. Here Bay comprises a diverse patient group with complex needs.
Your role involves working as part of the community nursing team visiting housebound patients in their own homes. Using your clinical expertise, you will be responsible for managing the patient caseload and coordinating the team to ensure safe, effective, and planned care.
Our district nursing teams are closely aligned with local Primary Care Networks to ensure holistic and coordinated patient care. We take time to get to know our patients, believing in treating the whole person, not just the condition.
District nurses provide complex nursing care to people at home and in residential care homes.
Your patients could be of any age, often elderly, recently discharged from hospital, terminally ill, or with physical disabilities.
As a Band 6 Caseload Manager, you will work as part of a team to keep people safe and well at home. You will use your clinical judgment to support evidence-based care planning for people living at home, in care homes, or other settings, ensuring a positive experience for service users.
