Do you have experience in supporting people? Do you know how to deliver a range of community activities? If yes, then we may have the role for you.

A fantastic and highly rewarding opportunity has arisen for a Relief Community Inclusion Facilitator to join our enthusiastic and passionate colleagues in Nottingham. This is a flexible contract that can work for you.

As a Relief Community Inclusion Facilitator, you will be a valued member of the team, flexible and committed to supporting people. Your role involves developing and offering a range of responsive, direct individual and group activities to people with a learning disability accessing the Me Time Programme.

Key Responsibilities
  1. Work in a way that enables the people being supported to make decisions and choices, and encourage trying new opportunities in line with the life each person chooses to lead.
  2. Ensure that the plans for the people supported are person-centred, with the individual fully involved in all aspects of planning and review.
  3. Be creative, dynamic, and adaptable to potential challenging scenarios related to the needs of the supported individuals.
  4. Ensure the safety and wellbeing of supported individuals, monitor their progress, and report on the impact of the programme on their lives.
  5. Complete all recording requirements promptly, maintaining high-quality reporting and case studies.
  6. Communicate with supported individuals with dignity and respect.
  7. Support colleagues in building community networks, sharing contacts and connections, and raising concerns through appropriate channels, especially regarding safeguarding.
  8. Be flexible and able to work from different locations and projects within the community.

The Me Time model is person-centred and focused on improving quality of life for participants, providing pathways to opportunities like volunteering or employment. It is a social inclusion, leisure, and community-led programme supporting people with learning disabilities to develop skills, confidence, and relationships within their local community through various activities and support to access community groups and opportunities.
