Job Summary

We are a large PCN comprising 11 practices and over 103,000 patients. This is an excellent opportunity to join our dynamic team of 12 pharmacists and 3 Technicians. The role is practice-based; we do not consider applications from individuals seeking remote positions.

This position allows you to work within an organization that employs most of our Pharmacists and Technicians directly, supporting their learning and development under the guidance of an experienced Senior Clinical Pharmacist.

Main duties of the job

To provide a Clinical Pharmacy Service to our GP practices, including:

  • Routine clinical medicines reviews.
  • Reviewing and reducing inappropriate poly-pharmacy and wasteful prescribing through timely medication reviews.
  • Reconciling medicines after hospital discharge and coordinating with patients and community pharmacists.
  • Developing referral processes with community and hospital pharmacy colleagues, promoting repeat dispensing services.
  • Managing the repeat prescribing reauthorization process.
  • Handling medicine queries from patients and healthcare professionals.
  • Implementing NICE and other evidence-based guidelines in collaboration with primary care professionals and patients.
  • Running clinics to meet PCN needs.
  • Contributing to multi-morbidity and multidisciplinary reviews.
  • Providing medicines information and training to PCN healthcare staff.
  • Serving as a medicines information resource for the team and patients.

About Us

We are a diverse PCN serving Nuneaton and Bedworth in North Warwickshire, with over 60 ARRS staff including Care Coordinators, Social Prescribers, and other health professionals, alongside our Pharmacists and Technicians. Our Senior Management Team supports various initiatives and digital innovations, enhancing patient care and staff development.

Our experienced team of Pharmacists and Technicians works across 11 practices, led by a Senior PCN Clinical Pharmacist. We value our employees and support their training and career growth.

Most Pharmacists work across two practices.

Salary: £47,475 to £50,640 annually, with higher rates for Independent Prescribers.

Location: Manor Court Surgery, 5 Manor Court Avenue, Nuneaton, Warwickshire, CV11 5HX

Job Responsibilities

Key Responsibilities:

  • Conduct medication reviews in high-risk groups such as the elderly, poly-pharmacy, renal/hepatic impairment, substance misuse, and patients on high-risk medicines.
  • Lead on prescribing practice improvements and respond to evidence changes affecting prescribing.
  • Coordinate with community pharmacy to support medication adherence.
  • Support clinical care improvements through audits and implementing changes.
  • Provide prescribing advice, ensuring patient safety during care transfers.

Additional duties include advising on self-limiting conditions, promoting cost-effective prescribing, reviewing new patient medications, managing repeat prescribing systems, supporting clinicians with dependencies, signposting patients, assisting with QOF targets, administering vaccines, educating the clinical team, ensuring compliance with guidelines, and acting as the PCN's medicines contact.

Person Specification

Qualifications (Essential):

  • Degree in Pharmacy and GPhC registration.
  • At least 2 years postgraduate experience, with 1 year in general practice.
  • Member of RPS.

Desirable:

  • Prescribing qualification.
  • Postgraduate pharmacy qualification.
  • Awareness of common conditions in general practice.
  • RPS Faculty membership or working towards it.

Other (Essential):

  • Full driving license and vehicle ownership.

Experience (Essential):

  • Experience in NHS and PCN service delivery.
  • Strong influencing and relationship skills.
  • Ability to work in multidisciplinary teams and communicate effectively with stakeholders.

Desirable experience includes supervising staff and empowering clinical professionals to improve service quality and cost-effectiveness.

Additional Requirements

This role is subject to a DBS check under the Rehabilitation of Offenders Act 1974. Applicants must hold current UK professional registration.
