Job description

Responsibilities and Expectations
  • Case managing residents and liaising with families, healthcare professionals, and other stakeholders to ensure the creation and delivery of optimal care plans.
  • Administering and recording medication accurately.
  • Carrying out comprehensive risk assessments.
  • Collaborating with care assistants to support residents with daily needs, promoting activity and independence.
  • Working closely with the leadership team to develop and improve services.
  • Maintaining a fun, inclusive, and professional environment at all times.
Leadership and Supervision
  • Leading and supervising your shift effectively.
  • Continuing responsibilities as above for resident care and stakeholder communication.

Qualifications and Skills
  • NVQ Level 3 in health and social care is desirable.
  • Well-organized, IT literate, and capable of delegating work appropriately.
  • Proven leadership and supervisory skills.
  • Good understanding of care planning processes.
  • Willingness to undertake training on administering medication if not already experienced.
  • A natural ability to enhance the lives of others.
