• Welsh speaker desirable.
• Evidence of ongoing professional training.

Responsibilities

• Provide clinically effective district nursing care within the District Nurse Team.
• Travel between patients' homes across towns and rural areas in a timely manner.
• Develop and work towards district nurse qualifications (Band 6/Chronic Condition Case Manager/Nurse Practitioner).
