Turning Point is seeking a COSUM Recovery Worker to support people with co-occurring substance use and mental health needs across the UK. A Full UK Driving Licence and access to a vehicle are essential.

The role offers a starting salary of £25,870 rising to £30,265 per year, with progression opportunities within a multidisciplinary team. You will develop personalised support packages, promote integration, and collaborate with recovery services to improve client outcomes.

Aufgaben

• Promote integration to prevent gaps in services for people with co-occurring needs
• Build partnerships with Drug and Alcohol Recovery services to enhance care pathways
• Coordinate joint working to identify, monitor and respond to needs of dual diagnosis individuals
• Monitor care pathways via case meetings and shared support plans
• Identify and coordinate longer-term support needs for individuals with dual diagnosis
• Ensure tailored exit and follow-up plans based on individual needs
• Provide staff training on dual diagnosis to ensure team competencies
