Join Our Community Nursing Team as a Band 7 District Nurse Team Leader

Are you looking to develop your leadership skills and be part of a dynamic Community Nursing Team within Harrow? We are seeking experienced professionals to lead and inspire our team in delivering high-quality community care.

1. Role Responsibilities:
   • Provide clinical leadership and manage the day-to-day operations of the District Nursing team.
   • Manage patient care referrals from GPs, aiming to prevent unnecessary hospital admissions through comprehensive assessments, care planning, and coordination with other agencies.
   • Communicate effectively with GPs, participate in meetings, and keep them informed about patient progress.
   • Maintain clinical credibility by adhering to evidence-based practices and continuously reviewing and improving clinical care.
   • Collaborate with the Clinical Development Lead to organize and develop training programs for District Nurses.
