This is an excellent opportunity for a highly motivated and enthusiastic Band 6 District Nurse to join the Adult Community Nursing Team, working flexibly over a seven-day period. The Specialist Practitioner Qualification in District Nursing is desirable or at least substantial years of experience working at band 5 level within the district nursing service.
The Evening and Overnight District Nursing Team delivers care between 18:00-08:00 across the community of Salford to people in their own homes. We are part of a dynamic, integrated health and social multidisciplinary team delivering high-quality care, advice, support, and monitoring to patients, their families, and staff within community settings. You will work under the supervision of the District Nurse Lead to provide care to patients with complex and non-complex needs, aiming to promote independence and self-management.
You should have excellent communication skills.

Your responsibilities include providing nursing care within the community, planning workload for junior staff, and supporting the team leader. You will participate in delivering nursing care and supporting clients in the community, working autonomously with confidence and competence. You will assess and manage the health needs of an identified population, including end-of-life care and patients with long-term or complex needs.
Strong communication and interpersonal skills are essential, as you will chair team safety huddles, attend multidisciplinary meetings, discharge planning meetings, and provide feedback to ensure seamless patient care.
You will be responsible for assessing patient care needs, developing, implementing, and evaluating care programs for specific patient groups, and supporting overall patient care. As a Sister/Charge Nurse, you will provide daily clinical and managerial leadership to nursing and support staff, promoting evidence-based practice.
The core hours of the role are:
covering a 7-day service.
