Mid Cheshire Hospitals NHS Foundation Trust seeks a Band 6 District Nurse/Case Manager to work in the SMASH Care Community Team in Sandbach. The role focuses on leading in delivery of person-centred, safe care for patients with complex needs, and involves close collaboration with patients and families.

You will manage district caseloads, coordinate nursing input, and respond to cross-cover needs across the Central Cheshire footprint.

Responsabilités

• Management and coordination of patients requiring community nursing input.
• Assessment of complex nursing care needs; develop, implement and evaluate individualised care plans for district patients.
• Deputise for the team leader in their absence to ensure service delivery across the district service.
