Job summary

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) in Sandbach is seeking a Band 6 District Nurse / Case Manager to join the SMASH Care Community Team. The role supports person-centred, safe care for patients with complex needs and end-of-life requirements.

You will work across the CCICP footprint, leading on caseload management and clinical leadership within the community nursing team, with travel as an essential part of the role.

Qualifications

• Excellent interpersonal and communication skills, written and verbal.
• Evidence of leading and managing staff and caseload management.
• Wide understanding/experience of community nursing.

Responsibilities

• Lead in the delivery of person-centred, safe and effective care.
• Manage care for patients with complex needs and end-of-life care.
• Develop, implement and evaluate programmes of individualised care.
• Coordinate community nursing with cross-professional teams; manage caseloads.
• Travel across the Central Cheshire Integrated Care Partnership footprint as required.
