Job summary

First Community Health & Care is seeking a motivated Band 6 NMC registered District Nurse to join the South Tandridge team, delivering NHS care to patients in their homes across a 7-day service.

You will manage a caseload, work with colleagues across neighbourhood teams, and support patients and carers to prevent hospital admissions while enabling self-care. Driving is essential for travel across the patch.

Qualifications

• RN1 registration and Specialist Practice Qualification in District Nursing are required.
• Mentor- Practice assessor and supervisor qualification is expected.
• Experience in district nursing and leadership within MDTs is preferred.

Responsibilities

• Manage and deliver high standard of cost-effective nursing care in the community.
• Maintain caseload of patients and provide care closer to home to avoid hospital admission.
• Assess, plan, implement and evaluate episodes of care.

Areyou a motivated and enthusiastic District Nurse with a Specialist PracticeQualification looking to continue developing you career providing NHS care?

Weare looking for a highly skilled and enthusiastic Band 6 NMC registered DistrictNurse to join our South Tandridge team.

Youwill need to be forward thinking, motivated and able to work collaborativelywith patients, their carers and health and social care colleagues.

Youneed to have a flexible approach to working some weekends and bank holidays asthe teams provide a 365 day service.

This role is part-time, 22.5hours per week (potentially 30 hours) across a 7 day a week service, with anexpectation to travel across the geographical patch.

Main duties of the job

Working as part of a team, you will be supported by the Team Lead to beresponsible for the continued management of a caseload of patients. You will beworking with other health care professionals across the neighbourhood, providinghigh standards of care to people in their own homes who have a wide range ofnursing needs; aiming to avoid hospital admission, promoting self-care andenabling people to be cared for in their preferred place.

As a District Nurse you will have line management responsibilities andwill support the Team Lead with theteams ongoing development and well-being.

Due to the nature ofthe role, it is essential that you can drive, have a car suitable for businesspurposes, and a valid driving licence.

Job responsibilities

The District Nurse is responsible for the management and delivery of a highstandard of cost-effective nursing care in the community.

You will be responsiblefor the continued management of a caseload of patients, working within aneighbourhood providing care closer to home aiming to avoid hospital admission.

You will ensure that patients receive safe, individualised, and effective standardsof care by assessing, planning, implementing, and evaluating episodes of care.

You will work as part of a highly skilled team, working closely with patients andcarers, linking with acute hospitals and primary care, actively encouragingeffective communication at all times with all relevant agencies.

You will promote and develop evidence-based practice in line with current policyand procedures and NMC code of conduct.You will line manage staff and be responsible for the development andcompetencies of staff within their team.

The service currently operates 365 days a year between 8am and 6pm and youwill be required to work shift patterns between these times.
