Liverpool City Council is seeking a motivated Hospital Discharge Assessment & Prevention Officer to join the Housing Solutions Service. You will identify individuals at risk of homelessness and take proactive steps to prevent homelessness, conducting housing assessments and developing practical solutions with partners.

Experience in housing or homelessness assessments.
