Creative Support seeks a Discharge Support Worker for our Mental Health Floating Support team in Greater Manchester. The role delivers housing-related discharge support within hospital wards and local outreach across multiple sites in North and South Manchester.

Experience with mental health needs is essential. We offer training toward a QCF Diploma and a mix of in-person and e-learning to develop your care career.

Qualifications

• This role requires experience in supporting individuals with mental health needs.
• Opportunity to train toward a QCF Diploma in Health and Social Care through our training system.

Responsabilités

• Provide hospital in-reach housing-related support to help people discharge from hospital.
• Coordinate with housing services and NHS partners to remove discharge barriers.
• Address barriers such as homelessness and financial issues to facilitate discharge.
• Deliver support across multiple sites in Greater Manchester, including local community outreach.
