Job summary

South West London and St George's Mental Health NHS Trust is seeking a part-time Discharge Coordinator (Band 6) to support the Acute and Urgent Care service line. The role focuses on proactive discharge planning, coordinating with wards, and addressing barriers to timely discharge, with coverage of Tolworth or Springfield Hospital.

You will work within internal and external partners to ensure timely access to services and promote smooth patient flow through discharge meetings and pathway

Qualifications

• Experience in discharge coordination or a similar administrative role.
• Ability to prioritise tasks to support timely discharge pathways.
• Strong communication and MDT collaboration across wards.

Responsibilities

• Admin and clerical duties for Band 6 Discharge Coordination.
• Cover Tolworth or Springfield Hospital wards as required.
• Prioritise activities to provide efficient, safe discharge pathways and escalate issues as needed.
