Main duties of the job
The Digital Transformation Manager will lead digitalinitiatives to modernise primary care services across Hillingdon's Primary CareNetworks (PCNs) and GP practices, promoting integrated care delivery. The roleemphasises enhancing patient access, optimising workflows, and fosteringcollaboration across the health and care system to improve efficiency, patientoutcomes, and population health management in alignment with NHS England'sdigital priorities and North West London ICS objectives.

Job responsibilities
- Leadthe planning, deployment, rollout, and optimisation of digital transformationprojects in Hillingdon primary care, with a focus on seamless integration anduser adoption.
- Supportthe deployment and ongoing PCN-level implementation of Blinx PACO (Patient andCare Optimiser), a cloud-based platform for online consultations, triage,patient communications, appointment management, and analytics.
- Providesupport to PCNs and practices in using EMIS Web, including systemconfiguration, troubleshooting, and promoting adoption to reduce administrativeburdens and enhance patient access.
- Workacross stakeholders to enable cross-working and integrated care, includingcollaboration with Central and North West London NHS Foundation Trust (CNWL)for community and mental health services, and The Hillingdon Hospitals NHSFoundation Trust for acute care pathwaysand being the digital lead into the ICB and national teams as and when needed
- SupportNHS App integration and rollout with Blinx PACO, ensuring patients canseamlessly link to online consultations and digital services via the NHS App asthe primary digital front door.
- Drivechange management, stakeholder engagement (including clinicians, practicemanagers, PCN leads, and partner organisations), benefits realisation, andmitigation of adoption barriers.
- Monitorproject deliverables, risks, and KPIs, such as improved patient engagement,reduced demand on traditional channels, and better proactive care.
- Identifyopportunities for digital innovation, including automation, total triage,neighbourhood team integration, and interoperability with existing systems.
- Support the research team toundertake complex searches as and when required and support in the digitaldevelopment within the research function
- Ensureadherence to NHS data security, information governance (including GDPR), cybersecurity, and accessibility standards.
- Contribute to resource planning,procurement, and reporting on digital initiatives.

Experience
- Proven track record in digital transformation within NHS primary care or integrated health settings (e.g., implementing online consultation tools, patient engagement platforms, or PCN projects).
- Excellent stakeholder management and communication skills, with experience engaging diverse groups including clinicians, PCN leads, community trusts (e.g., CNWL), and acute providers (e.g., Hillingdon Hospitals NHS Foundation Trust).
- Hands-on experience with primary care systems, particularly EMIS Web, and patient-facing digital tools.
- Deep understanding of primary care operational challenges, integrated care models, and demand management.
- Knowledge of NHS digital standards, interoperability, and patient access initiatives (including NHS App integrations).
- Good IT Skills
- Able to obtain and analyse complex technical information
- Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors when appropriate
