We have an exciting opportunity for a Band 5 Dietitian to join our Merton Community Nutrition and Dietetic team. You will be an enthusiastic, self-motivated individual who has an interest in developing skills in gastroenterology and nutrition support in a proactive department. The role supports patients with long-term chronic conditions and dietary education, including those at risk of malnutrition, adopting Food First guidelines and appropriate prescribing of Oral Nutritional Supplements. You will be providing a Dietetic Service in the London Borough of Merton, working closely as part of an MDT service alongside the Neuro Rehabilitation Team, as well as the wider Nutrition Support and Gastro workstream within CLCH.
To provide, under the supervision of your supervisor, a clinical dietetic service to adults in the community of Merton. This will mainly comprise of assessing and reviewing a caseload of patients requiring oral nutrition support in their own home and in care homes. Managing a small caseload of patients requiring Home Enteral Feeding in care homes. Carrying out regular virtual clinics within the community setting for patients with long-term conditions including gastro, neurological, and cardiovascular diseases.
