Job summary

DaVita is seeking a Licensed Social Worker to support dialysis patients across three clinics (Dover, Millersburg, Wooster). The role focuses on psychosocial health, patient advocacy, and education, with a team-oriented environment and flexible scheduling to meet patient needs.

You will manage a sizable caseload, collaborate with the care team, and provide individualized support for navigating the financial and emotional aspects of kidney disease. Strong interpersonal skills are essential.

Qualifications

• Meets all state required regulations to practice in dialysis settings.
• LSW licensure in the state of application.
• Master’s degree in Social Work (MSW) with clinical specialization.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook; proficient with DaVita applications within 60 days.

Responsibilities

• Provide psychosocial health interventions for dialysis patients and complete mandatory assessments, screenings, and education.
• Maintain caseload management across multiple clinics while supporting patient families.
• Build meaningful and long-term relationships with patients and families in an outpatient setting.
• Advocate for patients and guide them in managing financial, emotional, and medical aspects of End Stage Renal Disease.
