Job summary
An exciting opportunity has arisen for an experienced Registered Nurse to join the Integrated Diabetes Service. The focus of this role will be to work closely with our multidisciplinary team (MDT) to support adult inpatients across ELHT.
Our Integrated diabetes team is an early adopter of technology and uses examples of all current diabetes technology available to co-ordinate patient care, whether this is flash glucose monitors, insulin pumps and CGM.
The ELHT integrated diabetes service provides complex care to inpatients with diabetes across ELHT and BwD, the inpatient service is 7 days.
Main duties of the jobThe post holder will jointly manage a caseload of inpatient's whilst working alongside a highly skilled team of Adult Consultants, Adult Diabetes Specialist Nurses, Specialist Podiatrist and Specialist Dietitians. ELHT have been successful in receiving NHS England bid monies to develop our inpatient service. The role is to prevent unnecessary admissions, support inpatient care and timely discharge, and delivery training to HCP across the health economy.
