Health and Beyond in the United Kingdom seeks an experienced Diabetes Specialist Nurse to join our team. Based mainly in Birmingham, this full-time role involves delivering safe diabetic care across multiple sites and in patients' homes, working with the Diabetes team to improve health outcomes.

You will conduct autonomous assessments, run specialist diabetes clinics, and maintain robust call and recall systems.

Formación

• Registered Nurse at level 1.
• Post-registration diabetes-related course at level 3.
• Postgraduate qualification.

Responsabilidades

• Work as part of a multi-disciplinary team to deliver safe and high-quality care.
• Carry out annual review of diabetic patients, including medication review.
• Support the practice team in triage, assessment and treatment of diabetes patients.
• Prescribe and review medications within scope of qualified practice.
• Coordinate planning and delivery of care with patients and carers.
