Job summary

Surrey Downs Health & Care is seeking a dedicated Diabetes Specialist Nurse to join our community diabetes service. You will work closely with the diabetes team, dietitians and consultants to deliver high-quality nursing care and advance diabetes care across the partnership.

You will contribute to clinical governance, set standards with the multidisciplinary team, and help monitor patient outcomes while supporting operational delivery in your area.

Qualifications

Clinical practitioner with advanced diabetes care skills.
Leadership and collaborative working with multidisciplinary teams.

Responsibilities

Provide high-quality nursing care within the designated diabetes service area.
Work with DSNs, dietitians and diabetes consultants to deliver community diabetes care.
Assist in setting clinical standards and monitor patient outcomes.
Contribute to operational effectiveness in the designated clinical area.
Participate in education, research, training and development of the team.
