Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust is seeking a Diabetes Specialist Nurse to join the Community Diabetes Team on a permanent part-time basis (18.25 hours). The role involves delivering expert diabetes care across community and hospital settings, with emphasis on NICE guidelines, patient education and joint working with the Acute Trust.

Qualifications

• Excellent communication and interpersonal skills are required with a commitment to team working.

• Specialist in diabetes care with ability to assess, plan and implement care.

• Assess, plan and implement care for patients with diabetes, providing specialist advice.
