Job summary

Wirral University Teaching Hospital NHS Foundation Trust is seeking a Diabetes Specialist Nurse to provide highly specialist diabetes care, including antenatal diabetes support, across the patient pathway. The role involves advanced clinical practice, prescription responsibilities, and service development within the Medical Division.

You will work with a multidisciplinary team to lead diabetes care improvements, support insulin pump services, and deliver person-centred care for diverse patient

Responsibilities

Request and interpret diagnostic investigations; make clinical judgments and adjust treatment plans.
Acts independently as a registered Nurse Prescriber (V300) for patients with Diabetes mellitus, prescribing within an agreed clinical management plan.
Develop, produce and implement diabetes mellitus protocols which impact across primary and secondary care.
Provides highly specialist advice to independent trusts as required.
Provide one to one counselling to patients on life changing treatments, ensuring patients understand complex drug therapies and chronic disease management.
Provide a holistic, highly autonomous specialist nursing service to patients with Diabetes Mellitus assessing physical, psychological and social needs and implementing an agreed clinical management plan with the patient.
Support the provision of hybrid closed loop therapy and other technologies during pregnancy.
