Job summary

NELFT NHS Foundation Trust in London seeks a Diabetes Specialist Nurse (Band 7) to join our Waltham Forest community team. Role is permanent, based at the Jane Atkinson Centre, with salary £49,397 - £56,515 (pro-rated for part-time hours). Working Monday to Friday, 9am-5pm, to lead education and provide expert diabetes care.

You will join a multidisciplinary team and play a key role in managing a complex caseload, developing services and supporting patients to self-manage their condition.

Qualifications

  • Registered Nurse with active NMC registration and five years of nursing experience.
  • Post-graduate diabetes training at level 6 or 7 with structured education accreditation desirable.
  • Independent prescribing (V300) and a teaching or mentoring qualification; experience in community diabetes care.

Responsibilities

  • Provide patient education and empower self-management.
  • Lead service development in community diabetes care.
  • Collaborate with primary and secondary care colleagues to improve outcomes.

Working within a highly experienced community diabetes team, you will provide patient education, lead service development, and collaborate with primary and secondary care colleagues to ensure the best possible outcomes. Your clinical expertise and leadership will contribute to managing a complex and autonomous caseload. Provide tailored, structured education to empower patients and support self-management. Contribute to multidisciplinary team (MDT) meetings to develop holistic care plans. Exhibit advanced clinical skills in managing a diverse, complex patient population.

Essential Skills and Qualifications
  • Registered Nurse qualification with active NMC registration and five years of nursing experience.
